I was diagnosed with an enlarged prostate and have been taking one Tamdura capsule daily for about a year. My urination has improved considerably. Can taking Tamdura continuously cause complications or long-term side effects?
Quick Answer: Is Long-Term Tamdura Use Safe?
Tamdura can be used long term when it is appropriate for your BPH, but long-term treatment is not side-effect-free and should be reviewed periodically.
- In India, current Sun Pharma Tamdura capsules contain tamsulosin 0.4 mg + dutasteride 0.5 mg. Check your own package because brand formulations can vary by market.
- Tamsulosin improves urine flow relatively quickly by relaxing smooth muscle around the prostate and bladder outlet.
- Dutasteride works gradually, reducing DHT and shrinking enlarged prostate tissue over months.
- Important possible side effects include ejaculation changes, reduced semen volume, erectile dysfunction, reduced libido, dizziness and breast enlargement or tenderness.
- Tamsulosin can cause postural low blood pressure, lightheadedness or occasionally fainting.
- Dutasteride can reduce PSA by roughly 50%, so PSA results must be interpreted differently while you are taking it.
- Do not stop Tamdura simply because you feel better. BPH symptoms may return, and any change should be discussed with your prescriber.
What Is Tamdura?
Current Indian product information for Sun Pharma Tamdura lists two active medicines:
- tamsulosin hydrochloride 0.4 mg;
- dutasteride 0.5 mg.
These are two different classes of BPH medication working in complementary ways.
Tamsulosin: Faster Symptom Relief
Tamsulosin is an alpha-1 blocker.
It relaxes smooth muscle around the prostate and bladder outlet and can improve symptoms such as:
- weak urine flow;
- difficulty starting urination;
- straining;
- and other lower urinary tract symptoms.
However, tamsulosin does not significantly shrink the prostate.
Dutasteride: Slower Prostate-Reducing Effect
Dutasteride is a 5-alpha-reductase inhibitor.
It reduces formation of dihydrotestosterone (DHT), which contributes to prostate growth.
Over time, 5-alpha-reductase inhibitors can reduce prostate volume by roughly 18–28% and reduce the longer-term risk of acute urinary retention and BPH-related surgery in appropriately selected men.

Can Tamdura Be Taken for Years?
Yes, when there is a continuing medical indication.
Current EAU guidance specifically recommends alpha-blocker + 5-alpha-reductase-inhibitor combination treatment for men who have moderate-to-severe urinary symptoms and an increased risk of BPH progression, for example men with a larger prostate.
This is generally intended as long-term therapy, rather than a short course.
Four-year CombAT trial data showed that dutasteride plus tamsulosin provided better long-term symptom and urine-flow outcomes than either drug alone and reduced the risks of urinary retention and BPH surgery compared with tamsulosin alone.
Long-Term Does Not Mean “No Monitoring Needed”
The medicine may continue for years if it remains effective and tolerable, but symptoms, adverse effects and PSA interpretation should be reviewed periodically.
What Are the Main Side Effects of Tamdura?
Because Tamdura combines two medicines, it can produce adverse effects associated with either drug.

Can Tamdura Reduce Sexual Desire or Cause Erectile Dysfunction?
Yes.
Reduced libido and erectile dysfunction are recognized adverse effects of dutasteride-containing treatment.
In the four-year CombAT data used in current prescribing information, sexual adverse effects were more common with combined dutasteride + tamsulosin treatment than with either medicine alone.
The current U.S. prescribing information for the same dutasteride/tamsulosin combination lists the most common adverse reactions as:
- ejaculation disorders;
- impotence/erectile dysfunction;
- decreased libido;
- dizziness;
- and breast disorders.
Do Not Assume You Must Simply Live With Sexual Side Effects
If the medication controls your BPH urinary symptoms but significantly affects your sex life, discuss the balance with your prescriber.
Depending on prostate size, symptom severity and progression risk, another medication strategy may sometimes be reasonable. Do not change treatment on your own.
Can Tamdura Cause Little or No Semen During Ejaculation?
Yes.
Tamsulosin can cause ejaculation disorders, while dutasteride can also reduce semen volume and affect sexual function.
Therefore, some men notice:
- less semen than before;
- very little visible semen;
- or an orgasm with little or no ejaculate.
This does not mean that semen is becoming dangerously trapped inside the prostate.
But red or brown semen is a different issue. Tamdura should not automatically be blamed for visible blood in semen.

Do Sexual Side Effects Always Disappear After Tamdura Is Stopped?
They often improve, but I would not promise that every adverse sexual effect immediately disappears after discontinuation.
Current dutasteride/tamsulosin prescribing information notes that some sexual adverse reactions associated with dutasteride have been reported to persist after treatment is stopped, although the role of dutasteride in that persistence is uncertain.
This is another correction from the old page, which stated that all side effects could simply be reversed with additional medicines.
Does Tamdura Affect Blood Pressure?
Tamsulosin can lower blood pressure, particularly when changing from lying or sitting to standing.
Symptoms can include:
- dizziness;
- lightheadedness;
- weakness;
- or occasionally fainting.
This risk may be more important in older men or people taking other medicines that lower blood pressure.
High Blood Pressure Is Different
If your blood pressure has risen—for example from the 120s to the 140s systolic—do not automatically assume Tamdura caused it. Persistent hypertension deserves its own assessment and medication review.
If you develop recurrent dizziness or fainting, contact your clinician rather than simply stopping or altering the dose yourself.
Why Does Dizziness Matter More in Older Men?
Alpha blockers can contribute to postural hypotension.
Current EAU guidance notes an increased risk of falls and fractures among older men using alpha-blockers, likely related in part to blood-pressure effects.
Practical precautions include:
- getting up slowly from bed or a chair;
- being cautious during nighttime bathroom trips;
- reviewing other blood-pressure-lowering medicines;
- and telling your doctor about falls, fainting or persistent dizziness.
Can Tamdura Cause Depression, Irritability or Mood Changes?
This requires careful wording.
Irritability is not one of the principal common adverse effects of tamsulosin/dutasteride combination therapy, so a new personality or mood change should not automatically be blamed on Tamdura.
However, mood symptoms deserve attention.
In 2025 the European Medicines Agency reviewed finasteride and dutasteride safety. A direct causal link between dutasteride and suicidal ideation could not be established, but EMA recommended adding a precaution about possible mood changes to dutasteride information because of the common 5-alpha-reductase-inhibitor mechanism.
Seek Help for Significant Mood Changes
If you develop persistent depressed mood, depression, severe behavioral changes or suicidal thoughts while taking a dutasteride-containing medicine, seek medical advice promptly.
Suicidal thoughts require urgent assistance rather than waiting for the next routine BPH appointment.
Can Tamdura Cause Breast Enlargement?
Yes, although it is uncommon.
Dutasteride can cause breast enlargement, tenderness, swelling or nipple discomfort.
EAU estimates gynecomastia with 5-alpha-reductase inhibitors in roughly 1–2% of patients.
Tell your clinician if breast symptoms become troublesome.
A new hard breast lump, nipple discharge, skin change or one-sided persistent abnormality should be examined rather than assumed to be a harmless medication effect.
One of the Most Important Long-Term Effects: Tamdura Lowers PSA
Dutasteride can reduce serum PSA by approximately 50% after several months of treatment.
This does not mean that prostate-cancer screening or PSA follow-up should stop.
Instead, the clinician establishes a new PSA baseline and interprets future results in the context of dutasteride use.

What Does Tamdura Have to Do With Cataract Surgery?
Tamsulosin exposure is associated with intraoperative floppy iris syndrome (IFIS), which can complicate cataract surgery.
If you are planning cataract or glaucoma surgery, tell your ophthalmologist that you:
- currently take Tamdura;
- or have taken tamsulosin in the past.
Do Not Stop Tamdura Before Eye Surgery on Your Own
Inform the ophthalmologist and the doctor prescribing Tamdura. They should decide how your medicines are managed around the procedure.
Can Dutasteride Affect Fertility?
Dutasteride can alter semen characteristics in some men, including semen volume, sperm count and motility.
The clinical significance for fertility varies and is not completely predictable for an individual patient.
If you are trying to father a child, tell the prescriber before starting or continuing long-term dutasteride-containing treatment.
Can You Stop Tamdura Once Urination Improves?
Do not stop it simply because you feel better.
Improvement may mean that the treatment is working rather than that the underlying enlarged prostate has permanently disappeared.
Tamsulosin acts relatively quickly, while dutasteride takes several months to reach its full effect.
EAU notes that in some men taking combination therapy, the alpha blocker may eventually be discontinued after several months without a major worsening of symptoms.
But this is not appropriate for everyone—particularly men who initially had severe symptoms.
A Fixed Combination Makes This a Doctor-Led Decision
Because Tamdura contains both tamsulosin and dutasteride in one capsule, stopping only the tamsulosin component requires changing the prescription rather than simply altering the capsule yourself.
If medication is no longer adequate or side effects become unacceptable, the next step may be a different medicine or, in selected men, discussion of BPH procedures and surgery .
What Monitoring Is Recommended During Long-Term BPH Medication?
Current EAU follow-up guidance recommends reassessing men after medication is started and then periodically if treatment remains effective.
Depending on the situation, follow-up may include:
- review of urinary symptoms;
- discussion of adverse effects;
- IPSS or another symptom score;
- urine-flow measurement;
- post-void residual urine;
- blood-pressure review where appropriate;
- sexual and ejaculatory symptoms;
- and periodic PSA monitoring in appropriate men.
EAU recommends review after starting combination treatment, again around six months, and then generally annually when symptoms remain stable and no concerning complication develops.
If your symptoms worsen despite medication, the broader BPH treatment options should be reassessed rather than automatically increasing or adding medicines.
When Should You Contact a Doctor Promptly?
Seek medical advice promptly if you develop:
- fainting or severe persistent dizziness;
- complete inability to urinate;
- severe weakness or repeated falls;
- a persistent painful erection;
- a new breast lump or nipple discharge;
- significant depression or suicidal thoughts;
- repeated visible blood in urine;
- or a significant unexpected rise in PSA while taking dutasteride.

If your Tamdura is the current Sun Pharma formulation used in India, it contains tamsulosin 0.4 mg plus dutasteride 0.5 mg. I would still ask you to confirm the ingredients on your own package.
This combination is indeed designed for long-term BPH treatment in appropriately selected men. Tamsulosin improves urine flow relatively quickly, while dutasteride acts gradually to reduce prostate size and the risk of BPH progression.
However, I would correct the reassurance in the older answer. Long-term use is not completely free of meaningful side effects.
The main problems I would discuss with you are ejaculation changes, decreased semen volume, erectile dysfunction, reduced sexual desire, dizziness or low blood pressure when standing, and occasionally breast tenderness or enlargement.
You do not have to wait one or two more years to discover whether side effects can occur. They may appear earlier or later, and any troublesome symptom should be discussed when it occurs.
I would also pay particular attention to PSA. Dutasteride commonly lowers PSA by about half, so your physician needs to know you are taking it when interpreting every future PSA result.
If you develop major sexual side effects, mood changes, dizziness or another troublesome problem, do not simply accept it and do not discontinue the medication on your own. Your doctor can reassess whether you still need both components, whether another BPH medication strategy is more appropriate, or whether a procedural option should be discussed.
